    Initial Psychiatric Evaluation (60 minutes)

    Your provider conducts a comprehensive 60-minute evaluation of your history, symptoms, and goals in a safe, judgment-free setting — in person in Stoughton or via secure video.

    Personalized Treatment Plan

    Together we build a plan tailored to you, which may include medication management, therapy referrals, and lifestyle recommendations.

    Ongoing Medication Management & Follow-Up

    We stay with you through regular follow-up visits to monitor progress, adjust treatment, and support your long-term mental wellness.

How to Prepare in 5 Simple Steps

Schedule Your Appointment

Book online through our secure patient portal or call us directly. We offer both in-person appointments at our Stoughton office and telepsychiatry visits via secure video.

Complete Intake Forms

After scheduling, you will receive intake paperwork electronically through our patient portal. Please complete these forms before your first visit to ensure we make the best use of our time together.

Gather Your Information

Bring your insurance card, photo ID, a list of current medications (including dosages), and any previous psychiatric or medical records that may be relevant to your care.

Attend Your First Visit

Your initial evaluation typically lasts 60 minutes. We will discuss your concerns, medical history, symptoms, and goals for treatment in a safe, judgment-free environment.

Begin Your Treatment Plan

Together, we will create a personalized treatment plan that may include medication management, therapy referrals, lifestyle recommendations, and follow-up appointments.

What to Bring to Your First Visit

  • Valid photo ID (driver's license, state ID, or passport)
  • Insurance card (front and back)
  • List of current medications with dosages
  • Previous psychiatric records or evaluations (if available)
  • List of past medications tried (including what worked or did not)
  • Contact information for other treating providers
  • A list of questions or concerns you would like to discuss
