What Is Treatment-Resistant Depression and How Is Spravato Different?

If you have tried antidepressant after antidepressant and still feel stuck, please hear this first: it is not your fault, and you are not out of options. Many people in Massachusetts live with a form of depression that does not respond to the usual treatments. It even has a name — treatment-resistant depression. And today, there is a different kind of treatment, called Spravato, that is bringing new hope.

What Is Treatment-Resistant Depression?

Treatment-resistant depression (often shortened to TRD) is depression that has not improved after you have tried two or more antidepressant medications. This means you took the medication as prescribed, gave it enough time to work — usually six to eight weeks each — and still did not feel meaningfully better.

Living with TRD can be exhausting and discouraging. You may start to wonder if anything will ever help. You may feel let down by treatments that worked for other people but not for you. These feelings are understandable. But TRD is common, it is recognized by doctors, and it is treatable.

Why Do Some Antidepressants Stop Working?

Most common antidepressants work on brain chemicals like serotonin. They help many people. But depression is complex, and not everyone's brain responds the same way to the same chemical pathway.

Think of it like a lock and key. Standard antidepressants are one set of keys. For some people, those keys simply do not fit the lock. That does not mean the lock is broken — it means you may need a different kind of key.

How Is Spravato Different?

Spravato is the brand name for a medication called esketamine. It is given as a nasal spray, and it works in a completely different way than traditional antidepressants.

Instead of targeting serotonin, Spravato works on a brain chemical called glutamate. Glutamate helps your brain cells build new connections. For people with depression, this can help "reset" pathways that have become stuck. Because it works differently, Spravato can help even when other medications have not.

Here is what makes Spravato stand out:

  • It works on a different brain system than standard antidepressants
  • It can work faster — some people notice relief within days or weeks, not months
  • It is FDA-approved specifically for treatment-resistant depression
  • It is taken alongside an oral antidepressant, not instead of one

  • What Is a Spravato Appointment Like?

Spravato is taken in a certified healthcare setting, not at home. This is for your safety. Here is what to expect:

  • You use the nasal spray yourself, under supervision
  • You rest comfortably for about two hours afterward while we monitor you
  • You will need someone to drive you home, since you should not drive that day
  • Appointments happen on a schedule, usually twice a week at first, then less often

  • Some people feel a little dizzy, disconnected, or drowsy during the monitoring period. These effects usually fade before you go home. Our team stays with you the whole time.

    Is Spravato Right for You?

Spravato is not the first treatment we reach for, and it is not right for everyone. It may be a good option if:

  • You have been diagnosed with depression
  • You have tried two or more antidepressants without enough relief
  • You are ready to try a supervised, in-office treatment

  • The only way to know for sure is a thorough evaluation. We take time to understand your full history, your symptoms, and your goals before recommending any path forward.

A Word on Cost and Coverage

Many insurance plans in Massachusetts cover Spravato for treatment-resistant depression, especially after other medications have not worked. Coverage is not guaranteed and varies by plan, so we will help you check your benefits before you begin.

    You Deserve to Feel Better

For too many people — especially in African, Caribbean, immigrant, and other communities where mental health is rarely discussed — depression is carried in silence for years. If you have been suffering, please know that needing a different treatment does not mean you have failed. It means you deserve care that fits you.
