Substances are often used to manage overwhelming trauma symptoms including hyperarousal, intrusive memories, nightmares, emotional numbness, and anxiety. While substances may temporarily dampen these symptoms, they prevent proper processing of trauma and ultimately intensify PTSD symptoms.
Recognizing Trauma-Related Substance Use
Using substances to manage flashbacks, nightmares, or intrusive thoughts. Increased substance use after triggers or reminders of trauma. Difficulty feeling safe without substances. Using substances to numb emotions or avoid painful memories. Escalating use over time as tolerance develops.
Trauma-Informed Treatment
Effective treatment for co-occurring trauma and substance use must be trauma-informed — meaning it recognizes the impact of trauma, avoids retraumatization, and creates a safe therapeutic environment. Psychiatric treatment may include SSRI or SNRI medications for PTSD, medication-assisted treatment for substance use, and coordination with trauma-focused therapy.
